The curved follicle challenge
In patients with Afro-textured hair, the follicle is curved beneath the scalp surface — sometimes significantly so — even though the hair shaft emerges straight or with a gentle curve. During FUE extraction, the punch must follow this curve without transecting the follicle. This is significantly more demanding than extracting straight follicles and requires specific adaptations in punch size and technique.
Managing transection risk
Follicle transection — cutting through the follicle during extraction — is the primary risk in Afro hair FUE. Dr. Musbeh uses trichoscopy to assess follicle curvature before surgery and adapts his punch size (typically 0.9–1.1 mm rather than the 0.7–0.8 mm used for straight hair) to provide more margin around the curved follicle. This reduces transection risk at the cost of slightly larger extraction sites.
Implantation and curl pattern
Recipient site creation in Afro hair patients requires attention to the natural curl pattern of the hair. Grafts must be implanted at angles and in directions that will produce a natural-looking curl pattern as the hair grows. DHI technique can be particularly useful in Afro hair cases, as the implanter allows precise control of angle and direction at implantation.
